CHAPMANVILLE, W.Va. — Jerry Mayo Leslie, 68, of Chapmanville, was born February 19, 1945 in Prestonsburg, Ky., a son of the late Frank and Hester Woods Leslie. He departed this life Saturday, May 11, 2013, at St. Francis Hospital in Charleston.

Mr. Leslie was a retired warehouse foreman for Arch Coal.

Survivors include his wife, Deanna Caudill Leslie; one son: Brandon (Heather) Leslie of Chapmanville; three daughters: Angela (Edwin) Burton and Kandra Leslie, both of Murfreesboro, Tenn., and Rebecca (Bill) Vance of Chapmanville; four brothers: Bennett Leslie of Springhill, Tenn., Darrell Leslie and David Woods, both of Prestonsburg, and John Paul Leslie of Lexington, Ky.; grandchildren: Nathaniel, Kristian, Sam, Kaylee, Will, Anna and Emma, and one great-granddaughter: Myla

Services will be held at 1 p.m. Thursday, May 16, at Nelson Frazier Funeral Home in Martin, Ky., with Robert Wireman officiating. Burial will be in the Leslie Family Cemetery at Emma, Ky.

Visitation will begin after 6 p.m. Wednesday at the funeral home.
