TO: UNKNOWN MOTHER OR ANYONE CLAIMING TO BE THE MOTHER OF THE BABY BOY JOHN DOE, INFANT, BELIEVED TO HAVE BEEN BORN ON SEPTEMBER 25, 2009, AND LEFT AT LOGAN REGIONAL HOSPITAL ON SEPTEMBER 25, 2009
Notice is hereby given to the above named parties pursuant to West Virginia code §49-6-1 and §59-3-1 that a petition in the above captioned matter has been previously filed in the Circuit Court of Logan County, West Virginia.
Notice is further given that a Contested Adjudicatory hearing in this case is scheduled for December 8, 2009, at 2:15 p. m. before the Honorable Judge Roger Perry, located on the Third floor of the Logan County Courthouse, Logan, West Virginia, to determine the issues alleged in the petition and the possible termination of parental rights in and to said child.
A copy of the Petition in this matter may be obtained from the LOGAN COUNTY CIRCUIT CLERK'S OFFICE, LOGAN, WEST VIRGINIA.
The parents of this child have the rights to counsel at every stage in these proceedings, and if not already appointed an attorney, one will be appointed for them upon application to the Clerk of this Circuit Court.
Failure to respond to this notice could result in possible termination of parental rights to this child.
